RG Pitampura Hospital has reinforced its expertise in minimally invasive gynaecologic surgery by successfully managing a rare and life-threatening case of ruptured bilateral ovarian endometriotic (chocolate) cysts complicated by haemoperitoneum and hemorrhagic shock.

A 32-year-old unmarried woman arrived at the emergency department with severe abdominal pain and worsening nausea over two days. She presented with dyspnoea, tachycardia and acute abdominal tenderness. Investigations confirmed ruptured endometriotic cysts on both ovaries with massive internal bleeding of nearly 1.5 litres, leading to hemorrhagic shock.

With Minimally Invasive Gynaecology & Robotic Surgery, the patient underwent an emergency laparoscopic surgery. Surgeons identified a ruptured right ovarian endometriotic cyst measuring 15 × 12 cm and a left cyst measuring 6 × 5 cm, along with dense omental and bowel adhesions that had completely obliterated the pouch of Douglas. Endometriotic implants were also found extending up to the liver surface.

Doctor’s performed laparoscopic excision of both cysts, peritoneal lavage, adhesiolysis and right ovarian reconstruction, successfully preserving the patient’s ovarian function and future fertility prospects. The patient recovered well postoperatively and was discharged in stable condition.
